1) Screening Form
This form is for the prospective participant to fill out and asks personal questions to determine if this group is the right fit. This information will be kept private and we will follow up with you if we have further questions.
Please have the child complete this form (with adult help if needed)
2) Information & Permission Packet
This form goes into detail about the workshop description, safety, liability, informed consent & therapeutic group agreement.
Parent/caregiver(s) & child are asked to read over and sign.
3) Complete Payment or Donation
Pay a $25 (non-refundable) deposit to hold your spot in the group OR pay your total payment all at once. If you are unable to pay the $25, email us at

4) Attend the Orientation Meeting Prior to Group Start
Each workshop will have its own orientation meeting close to the start date of the workshop. We ask all participants and parent/caregiver(s) to attend to receive vital information and ask any questions.
These meetings are typically held via Zoom on an evening prior to start date. You will be provided details for this meeting upon sign up.
